How do I make a "cover sheet" for a scholarship application? - fax cover sheet format

I need a cover for an application for a grant to create, but I have no idea how. I searched the Internet for the formats, but I have received the fax cover sheets and envelopes for classes.

Moreover, what is a cover? Does not mean that a letter of intent? I'm so confused!
